The Clinton School District utilizes Privit, an online system that allows us to ensure the health and safety of our athletes, as well as organize and protect the privacy of sports physical forms.

The district no longer accepts paper copies of physical forms. Parents of CHS and CMS athletes and marching band members will follow the instructions (link to the left) to create a Privit account. After creating an account, parents are able to print a physical form to take to their doctor. The completed form can then be uploaded to the Privit account just by taking a picture of the document. The entire process will take approximately 10 minutes, and there is no charge to parents.

Please remember that a Privit account must be created and physical uploaded before your child can participate in practice.
